Certified Birth Record Application
The Birth Certificate Application is required in order to obtain a certified birth certificate issued through the Minnesota Department of Health. Before returning the above application, please have your signature notarized, and include a check or money order for $26 made payable to the Chippewa County Recorder. If paying by credit or debit card, please include requested information on the application. Credit card fees may apply. Return the above to the following address:

Chippewa County is able to print birth certificates for those born in the State of Minnesota. If the subject was born outside of the State of Minnesota, they would then need to contact the appropriate state or country for that record. Certified birth records are available from 1935 and forward. If the date of birth was prior to 1935, the record must be obtained from the county in which the subject was born.
Additional Copies
Additional copies of certified birth records are $19 at the time of purchasing the first record.
Youth Experiencing Homelessness Birth Certificate Application
The Youth Experiencing Homelessness Birth Certificate became effective January 1, 2022. A homeless youth may apply for their birth certificate at no charge. This birth certificate is an official legal document and expires after six months from the date issued. The applicant must be 24 years of age or younger and born in Minnesota. These birth certificates may be requested at any Minnesota Vital Records Office by filling out the Youth Experiencing Homelessness Birth Certificate Application.
Certified Death Record Application
The Death Certificate Application is required in order to obtain a certified death certificate issued through the Minnesota Department of Health. Before returning the above application, please have your signature notarized, and include a check or money order for $13 made payable to the Chippewa County Recorder. If paying by credit or debit card, please include requested information on the application. Credit card fees may apply. Return the above to the following address:

Chippewa County is able to print death certificates in which the death occurred in Minnesota. If the death occurred outside of the State of Minnesota, please contact the appropriate state or county for that record. Certified death records are available from 1997 and forward. If the date of death was prior to 1997, the record must be obtained from the county in which the death occurred and the application must be sent there.

Additional copies of certified death records are $6 at the time of purchasing the first record.
