Developmental Disability Services
About Disability Services
Our Developmental Disabilities workers provide services to eligible children and adults with developmental disabilities. Our mission is to assist persons with disabilities to find the unique and individualized supports needed to live self-determined lives within their chosen communities.
Services include case management with experienced social worker case managers. Each eligible individual is assigned a case manager, who will help that individual to develop an individual service plan, provide information regarding available services and resources, and assistance in obtaining the funding for the services/supports that are chosen.

Developmental Disabilities (DD) Waiver

Family Support Grant (FSG) Program
The requirements for the Family Support Grant Program include:
- County application process
- Live or will live in their biological or adoptive family home
- Must be deemed certified disabled by County DD Screening, State Medical Review Team (SMRT), or Social Security Administration
- MA not required
- Limited budget available annually if eligible
- Adjusted gross income limit
- Eligibility and need determined annually
- State funds
